It is important that we obtain personal and financial
information on your family to assist you in your planning efforts.We will review this information in detail
prior to our next meeting.
Here is a listing of information that may be important
to your planning process. This data can
be itemized on the bottom of this page or on additional sheets as necessary.

Copies of your last Federal and State tax
returns

All current life insurance policies, disability
policies and annuity contracts. Copies
of latest annual statement if available.

Information regarding benefit programs provided
by your employer such as life, health and disability insurance, profit sharing,
retirement, or stock plans.

List of all debts. Show amount owed, payment, and interest rate.

If you own a home, provide information as to its
market value, mortgage balance, interest rate, and term of mortgage.

Recent pay stub for you and your spouse.
š
Copies of latest investment statements (stocks,
mutual funds, CDs, real estate, etc.) showing purchase date, price, current
value and monthly contributions.

Other assets, such as: valuable collections, lake property, raw
land, business interests, etc

Information and latest account statements for all IRA, TSA, or Keough plans.

Copy of current wills, trust documents, and
powers of attorney.
